In our discharge preparation department, young people are on their way to a more independent life. The department is part of the youth home but is located in housing outside the youth home's area. Young people belonging to our discharge preparation departments will live and spend their leisure time in apartments or other forms of housing outside the youth home. The young people attend external schools or have internships. Treatment activities mainly consist of skills training aimed at preparing the youth for a functioning life after their time at SIS, both practically and socially.
The role includes coordination and follow-up of weekly meetings, parent meetings, treatment interventions, and individual activity schedules. The treatment coordinator is responsible for the working method regarding motivational interviewing (MI), plans MI conversations, acts as an MI coach, and provides feedback to colleagues regarding journal notes as well as handling assessment forms and documentation. The assignment also includes conducting individual conversations, participating in treatment collegiums, collaborating on treatment plans, and planning and following up on ADAD during admission, transfer, and discharge.
